AcademyHealth envisions a future where individuals and communities are made healthier by the use of evidence in decision-making. Many types and sources of data are needed to support this vision, but these data must be shared with those who can use it to improve individual and population health. It is with this vision and motivation that we are excited to announce the National Call for Applications for theCommunity Health Peer Learning Program (CHP).
