Illinois Care Coordination Innovations Project
Illinois Governor, Pat Quinn, has called on health care providers to submit innovative coordinated care proposals that feature preventative medicine, new technologies, community outreach, and enhanced follow up treatment to improve the Illinois Medicaid program andpatient outcomes. The state Department of Healthcare and Family Services (HFS) has published a formal solicitation calling for provider proposals under the new Care Coordination Innovations Project.
