Building Medical Homes: Lessons from Eight States with Emerging Programs
The National Academy for State Health Policy and the Commonwealth Fund have released a report highlight 8 states (Alabama, Iowa, Kansas, Maryland, Montana, Nebraska, Texas, and Virginia) focused on developing and implementing the medical home program. The report details the selected states' experiences convening stakeholders, helping primary care practices improve performance, and addressing antitrust concerns that arise when multiple payers come together to create a medical home program.
