VT- Health Affairs, Vermont’s Blueprint For Medical Homes, Community Health Teams, and Better Health at Lower Cost
The Vermont Blueprint for Health is a statewide public-private initiative aimed at transforming health care delivery, improving health outcomes, and enabling the citizens of Vermont to recieve seemless, coordinated care. The pilot programs provide community health teams working with primary care providers to coordinate patient-centered care. The pilot programs have seen significant year-over-year decreases in hospitatl admissions and emergency department visits, and their related per person per month costs. The pilot programs hope to roll out statewide and project to save 28.7 percent in incremental health spending in the state by its fifth year.
